Hotels and most restaurants will have 10% and 5% (respectively) "service charge". This is the tip and should be divided up amongst all staff (including gardeners and the like) but excluding contract workers (eg, singers) and management. Unfortunately not all places do divide up the service charge, but most of the larger hotels do.
That being said, feel free to tip musicians and the waiters and waitresses who serve you. 10% is a good rule of thumb. Tip them more if you like the service you've received. Round up the rupiah to the nearest 10,000.
Alternatively you can hand over envelopes with an amount covering your whole stay to each staff member who has cleaned your room, for example. You'll be remembered for the next trip, that's for sure!
